Medical Transportation Services provides transportation to our three medical locations, Hu Hu Kam Memorial Hospital, Komatke Health Center and Ak Chin Clinic, in addition to medical offices and hospitals within Arizona.  Please make your transportation requests NO later than 2:00 pm the business day (Monday-Friday) prior to your scheduled appointment. Discharge transports are serviced by Gila River Medical Transportation. Please have the discharge nurse call HHK Emergency Department at (602) 528-1384 to arrange transportation home if necessary.

Important Tips and FAQs

To arrive to your appointment on time, please :

  •           >Ensure all personal/contact information is current and correct
  •           >Inform the Medical Transportation Clerks of any date or time changes immediately. This can affect your transport.
  •           >Be ready one hour to an hour an a half prior to your pick-up time. The driver can only wait 10 minutes after arrival. Being ready                upon driver                    
  •           >arrival, helps you and others get to medical appointments on time.

Department FAQs

Q. What are the eligibility requirements to obtain transportation to a GRHC facility / Clinic?

You must be CHS (Contract Health Services) eligible or covered by AHCCCS / AIHP (American Indian Health Program). If you are a Direct Care patient, you must be covered by AHCCCS/AIHP. All patients must reside within Contract Health Service Delivery Area (CHSDA). If you are unsure of your eligibility status and/or have questions, please contact the GRHC Medical Transportation Clerks.

Q. What information do I need to provide to Medical Transportation Services to make a trip request?

Your appointment must be a covered medical service under AHCCCS/AIHP (American Indian Health Program) and/or Contract Health Services (CHS). AHCCCS/AIHP does NOT cover dental exams, Podiatry or Audiology exams for patients 21 years or older. If there is a change in you phone/contact number and/or address for pickup/drop off, please advise the GRHC Medical Transportation Clerks so that there is no delay in your transportation.

Q. How far in advance must I make my transportation request?

Transportation requests must be made by 2:00 pm the business day prior to your scheduled medical appointment. This allows us time to process your request.
