The Public Health Nursing (PHN) Department proudly serves the Gila River and Ak-Chin Indian Communities, providing compassionate, culturally sensitive care across all stages of life—from newborns to elders. Our PHN team includes Registered Nurses, Licensed Practical Nurses, and Community Health Representatives (CHRs), each playing a vital role in delivering personalized care. Services include home visits for wellness assessments, one-on-one health education, chronic disease management, and limited assistance with medication organization.

The goal of the PHN program is to enhance the overall quality of life and functional ability of our patients by addressing medical needs with respect, empathy, and clinical expertise. We provide secondary and tertiary care directly in homes, at district service centers, PHN offices, elder wellness centers, and during community events—ensuring accessible and responsive support throughout the community.

The CHR Program focuses on promoting the health and well-being of both the community as a whole and its individual members. Our approach is centered on three core goals: health promotion, disease prevention, and health maintenance. CHRs are frontline public health workers who provide: outreach, education, informal counseling, social support, patient-centered care, and advocacy services. They work to reduce barriers to healthcare, address social drivers of health, and improve access to critical services.
