The Case Management Department is the advocate for individuals and families’ comprehensive healthcare needs through communication and support using available resources to promote patient safety, quality of care, and to make it a smooth healthcare transition.
- Coordinate communication among your healthcare providers
- Patient Advocacy
- Resources to GRIC and other outside services
- Provide patients with education about diseases, services, and follow-up care
Case Management Staff:
RN Case Managers: Advocates and educates patients during their outside hospital admission. Collaborates with outside hospital staff for patient discharge needs.
Patient Liaison: Provides bedside assistance to hospitalized patients in external hospital facilities by providing resources to counteract issues that negatively impact their healthcare.
RN CARE COORDINATOR: Will work with the providers at Gila River Health Care to plan and coordinate your care.
Medical Social Worker: Help navigate the healthcare system, connect clients with community resources
