The Public Health Nursing (PHN) Department attends to members of the Gila River and Ak-chin Indian communities and serves patients across the entire life span (newborn to elderly, pregnancy though post-partum). Each team members provides home visits related to wellness assessments, one-one health education with disease management, and select medication organization assistance. Our goal is to promote access to preventative services such as immunization administration and health screenings. Our family nurse practitioner (medical provider) enhances the department by providing primary, secondary and tertiary medical management for Community members in their homes, at district service centers, PHN offices, elder wellness centers, and community events. Our PHN Department continues to strive and maintain community trust by being the community’s first responders when it comes to their routine health care needs.
