Who Can Help When You’re Admitted to an Outside (Non-GRHC) Hospital?

ANSWER: The GRHC Case Management Team can!

When GRHC patients are admitted to an outside acute care hospital partner such as Chandler Regional, Dignity, or Banner, the GRHC Case Management team goes into action to help patients and their families.

HOW DOES THIS HAPPEN?

Once a GRHC patient is admitted to a partner hospital, the Case Management team is informed by the partner hospital.

WHEN ADMITTED TO AN OUTSIDE HOSPITAL, GRIC PATIENTS ARE CONTACTED BY GRHC CASE MANAGERS:

  • To ensure the GRIC member/patient is receiving excellent care
  • To communicate on behalf of the patient/family with the partner hospital’s case manager, social worker, discharge planner, or care coordinator
  • To ensure discharge from the hospital is as stress-free and as easy as possible
  • To call GRIC patients after discharge to help with setting up a follow-up appointment with their GRHC Primary Care Provider

Often times, navigating outside healthcare services can be challenging. The GRHC Case Management Team exists to help patients while they are in the hospital and upon discharge, to ensure that their GRHC Provider Team is informed and prepared for follow-up treatment.

Overall, the Case Management Team is committed to ensuring that GRIC patients receive quality healthcare.

The Case Management department works with outside hospital staff at hospitals located outside of the Gila River Community to ensure that Community members’ needs are met and all arrangements are made prior to discharge.  Case Managers and Patient Liaisons make every effort to contact patients during their hospital stay or soon after discharge to ensure that needs have been met.
